Account closure form
This form authorises Origin to arrange a final meter reading to close the electricity account in the builder's name and to set up the account in the customer's name. The form enables the new home owner to request Origin to contact them to arrange electricity or gas supply for their home. The form must be filled in completely and signed by the customer for Origin to action the request. The customer is not locked into a contract by signing this form.
Electricity final read request
This form authorises Origin to arrange a final meter reading and close the electricity account in the builder's name. If the new home owner does not contact Origin to set up a new account, the electricity supply may be disconnected.
