Blanket application
Distribution network companies have regulated charges for services they provide as part of the connection process. By signing this application the builder acknowledges that Origin will pass these charges directly to your company. This agreement will ensure that our new connections team will not need to contact you for each connection you request. This will save time and the inconvenience of us needing to explain this prior to issuing work to the distribution network and seeking your approval on each occasion.
Electrical Works Request (EWR)
This form is to be submitted to Origin by the electrical contractor to arrange connection of electricity supply. The EWR must be accompanied by the Certificate of Electrical Safety (CES) before a Service Order can be raised and forwarded to the Distribution company to schedule the works.
Natural gas connection request
Request for a natural gas service line to be run into a property.
Natural gas meter fix request
Request for a natural gas meter to be installed at the property.
Account closure form
This form authorises Origin to arrange a final meter reading to close the electricity account in the builder’s name and to set up the account in the customer's name. The form enables the new home owner to request Origin to contact them to arrange electricity or gas supply for their home. The form must be filled in completely and signed by the customer for Origin to action the request. The customer is not locked into a contract by signing this form.
Electricity final read request
This form authorises Origin to arrange a final meter reading and close the electricity account in the builder’s name. If the new home owner does not contact Origin to set up a new account, the electricity supply may be disconnected.
Natural gas meter and service line abolishment form
The Natural Gas Meter and Service Line Abolishment form is used to apply for removal of gas services at the property.
